List with a vårdcentral as soon as you are registered
You choose your primary care centre and list with it, free, online with BankID or on paper. Doing it before you are ill means you have a named point of contact rather than starting from an emergency department.

Call 1177 before going anywhere
1177 is answered by registered nurses around the clock. They assess, advise and direct you to the right level of care. Turning up at Skånes universitetssjukhus without doing this is the classic newcomer mistake and produces very long waits.

Establish which country's social insurance you are in, in writing
If you work for a Danish employer you are normally covered by Danish social insurance rather than Swedish — but working 25% or more of your time in Sweden flips you into the Swedish system. This determines sickness benefit, parental benefit and healthcare entitlement. Ask Försäkringskassan for a determination rather than assuming, and use Øresunddirekt if you want it explained face to face.

Know the Skåne fees before you need them
Region Skåne is one of the cheaper regions for primary care: a doctor at a vårdcentral is SEK 200. Specialist and emergency care cost more. Everything counts towards a SEK 1,450 high-cost ceiling over a rolling twelve months, after which a frikort makes further outpatient care free — and the frikort is valid across the whole of Sweden.

Register children and use the free child healthcare
Healthcare is free for children and young people in Region Skåne, and BVC child health centres provide structured developmental checks from birth. It is one of the strongest parts of the system and it is opt-in by registration.

Cover the gap between arriving and being registered
Before you are entered in the population register you are not yet in the system. EU/EEA arrivals use the EHIC; everyone else needs private cover for that window, which can run several weeks.

Things most newcomers don’t know

Working 25% of your time from home in Malmö can move you out of Danish social insurance entirely.

Under EU Regulation 883/2004 a person living in Sweden and employed in Denmark is normally covered by Danish social insurance — but if 25% or more of the work is performed in the country of residence, the country of residence's system takes over the whole employment. That changes sickness and parental benefit, and it obliges the Danish employer to deal with the Swedish system. A hybrid arrangement of one day a week at home is inside the limit; two is not. This is a threshold people cross without noticing.

Denmark refused the EU telework agreement, so the generic European advice does not apply here.

The 2023 EU framework agreement lets cross-border teleworkers work up to 50% from home and stay in the employer country's social security — but only if both states have signed. Sweden signed with effect from 1 July 2023; Denmark announced in October 2023 that it would not, and the Belgian depositary's list of signatories still does not include it. What covers Öresund commuters instead is the narrower bilateral Öresundsöverenskommelsen, which is not the same thing and has its own conditions.

Region Skåne's fees are its own, and the SEK 1,450 ceiling is the only part that is national.

The statutory maximum high-cost ceiling is SEK 1,450 across Sweden and a frikort earned in Skåne works everywhere in the country. What varies is what you pay to get there — Skåne is among the cheaper regions for a GP visit at SEK 200, and a colleague's figure from Stockholm or Gothenburg will not match. Ask which region a quoted price came from.

The care guarantee gives you a legal right to go elsewhere when the wait is too long.

Sweden's national care guarantee sets maximum waiting times for contact, assessment and treatment. If your region cannot meet them it must offer care with another provider — including in another region — at no extra cost to you. Regions do not volunteer this and the onus is on the patient to ask. It is the most valuable and least used right in the Swedish system.

Common mistakes to avoid

Assuming your healthcare and social insurance follow your address — for a commuter they follow where the work is performed.
Drifting past 25% of working time in Sweden without telling anyone, and changing your insurance country by accident.
Relying on generic EU telework guidance that assumes Denmark signed the framework agreement.
Assuming Stockholm's or Gothenburg's patient fees apply — Region Skåne sets its own.
Arriving without cover for the weeks between landing and being registered.
