NFZ cover, the family form nobody files, and private packages

An employment contract enrols you in NFZ automatically through employer ZUS contributions. A B2B contract does not: you pay the health contribution yourself, at a rate that depends on which tax regime you elected. Public primary and emergency care works; the specialist queue is the weak point, which is why an employer private package is close to a default benefit in Wrocław's banking and R&D sector.

Things most newcomers don’t know

Insurance is checked electronically against your PESEL and nothing else.

The eWUŚ system returns a live green or red against your number when you present at a clinic. There is no card to show and no argument to make at the desk. It also means an upstream administrative failure — a late employer filing, a dependant never registered — presents to you as simply being uninsured, with the fix somewhere else entirely.

Your spouse and children need a separate ZUS form, and it is usually forgotten.

Family members are covered only after the employer files a ZCNA registration for each of them. Nothing prompts it and nothing confirms it, and the omission surfaces when a child is turned away at a paediatric clinic. Asking HR to confirm the filing takes one email and removes a genuinely stressful failure mode.

Your health contribution depends on the tax regime you chose, which surprises B2B contractors.

Since the 2022 reforms the health contribution for self-employed people is income-based rather than flat, and it is calculated differently under the progressive scale, the 19% flat rate and ryczałt. Under ryczałt it steps up in revenue bands and half of it is deductible. People compare income tax rates between regimes and forget that the health contribution can move the answer.

Wrocław's private clinic networks are thinner than Warsaw's, so the tier matters more.

The same package name buys a smaller physical network here. A tier that gives good access in Warsaw can mean a single Wrocław location with a three-week wait for the specialist you actually need. Ask for the clinic list for your city and your tier before treating private cover as a solved problem, particularly for paediatrics and physiotherapy.
