Ambato has a teaching hospital and an IESS hospital and they handle the ordinary run of medicine well. Two hours from Quito is the best referral position of any non-capital city in Ecuador.
Every smaller Ecuadorian city sends its complex cases to Quito or Guayaquil. What differs is how far. From Manta or Machala that is three or four hours by road; from Cuenca it is a long day or a flight. From Ambato the country's deepest concentration of specialist medicine is a two-hour drive on a main road. For anyone weighing a smaller city against a capital on health grounds, this is the concrete number that should be in the comparison, and it rarely is.

The sierra's health risks are the exact inverse of the coast's, and neither set transfers.
At 2,577 metres there is no dengue, no chikungunya and no Zika, because the vector does not live at this altitude — a permanent, structural advantage over Guayaquil, Manta and Machala that nobody mentions. What you get instead is altitude adjustment, extreme ultraviolet through thin dry air, and cold unheated buildings.
