Health🇳🇱 Rotterdam, Netherlands

Buying your own compulsory insurance, and finding a huisarts who will take you

There is no employer plan and no free service at the point of need. Within four months of becoming insurable you must buy a basisverzekering from a Dutch insurer, and it backdates to the date you became insurable, so a late purchase means paying for months of cover you could not use. The basic package is defined by law and identical at every insurer; only price, service and add-ons differ. On top is a €385 own-risk excess in 2026. Then you need a huisarts, who gatekeeps essentially all other care — and in Rotterdam that is harder than buying the policy.

Total cost
Roughly €142-185 a month for the basic policy in 2026, with the market average around €159, plus up to €385 a year of own risk if you use non-exempt care, less zorgtoeslag if you qualify. Adult dental sits almost entirely outside the basic package and is paid privately or through an add-on.
Time needed
The insurance itself can be arranged online in an afternoon. Finding a huisarts who will accept you can take weeks in Rotterdam and is the genuine bottleneck.
Validity
Policies renew automatically each January. You may switch insurer once a year by cancelling before 31 December for a 1 January start. The excess resets each calendar year. The €385 figure has been frozen for years; the coalition plan to halve it to €165 from 2027 was never enacted and lost its political majority, while an increase has since been floated, so confirm the figure for the year you are in with your insurer or Rijksoverheid rather than assuming.

High confidence·Anyone living or working in Rotterdam who is insurable under Dutch law. Insurance is national and private; the shortage of GPs accepting new patients is regional and worst in parts of Rotterdam-Zuid.

Before you start

  • A BSN from gemeente registration
  • A residence permit or EU registration that makes you legally insurable — you generally cannot insure while a permit decision is pending
  • A Dutch IBAN for the monthly premium direct debit
  • DigiD, to claim zorgtoeslag and to use most patient portals

Step-by-step

  1. 1

Buy the basisverzekering, backdated, inside four months

Compare on price, service and contracted hospitals only — the covered treatments are set by law and are the same everywhere. Set the start date to the day you became insurable, not the day you sign. Decide separately whether an aanvullende package for dental, physiotherapy or glasses is worth it, since the basic policy excludes adult dental almost entirely.

Register with a huisarts the week you have an address

Practices take patients only from their own catchment postcodes and many across Rotterdam are closed to new registrations. Search by postcode, call rather than emailing, and take the first practice that says yes even if it is not the nearest. Ask explicitly whether consultations in English are available.

  3. 3

Understand the eigen risico and what it does not cover

The first €385 of most care in a calendar year comes out of your pocket. GP consultations, maternity care and care for under-18s are exempt; prescriptions, lab tests, scans, specialists and ambulance transport are not. The excess resets on 1 January, which is why people cluster elective referrals.

Claim zorgtoeslag if your income is under the threshold

The healthcare allowance is paid monthly in advance by Dienst Toeslagen through Mijn Toeslagen with DigiD, and is reconciled against your actual year-end income. It can be claimed retroactively for the current year. It is means-tested on both income and assets, and plenty of eligible lower earners simply never apply.

Learn the out-of-hours routing before you need it

During surgery hours, call your own huisarts. Evenings, nights and weekends, call the Huisartsenposten Rijnmond and be triaged by phone — not the hospital. Call 112 only for genuine emergencies. Turning up at an emergency department unreferred for a non-emergency is discouraged and can leave you with a bill against your excess.

Things most newcomers don’t know

Insuring late does not save you money — the policy backdates and you pay for cover you never had.

The obligation starts the day you become insurable, usually your residence permit or registration date. Buy at month three and the insurer charges premiums from month zero, while nothing you spent in that gap is reimbursed. You get the worst of both: retroactive premiums and no retroactive cover. On top, the CAK can impose a fine and then arrange insurance for you at a worse price.

Every insurer sells the identical basic package, so a more expensive basisverzekering buys you nothing extra.

The Dutch government defines the covered treatments, and insurers may not vary them. What varies is the premium, the service, whether your preferred hospital is contracted, and whether the policy is a restitutie type that reimburses any provider or a natura type that pays only contracted ones. People routinely pay a premium for a brand name in the belief they are buying better cover. They are not.

In Rotterdam the scarce resource is a GP, not a policy, and catchment areas are enforced.

Practices accept patients only from defined postcodes, and shortages are acute in parts of Rotterdam-Zuid where list sizes are largest. If you register with a practice and then move across the river, you generally have to start the search again. Register the week your address is fixed, take whichever practice accepts you, and treat a nearby practice as a luxury rather than a given.

Erasmus MC changes the calculation for anything serious, and referral routing is worth knowing.

Rotterdam has the country's largest university medical centre and the Sophia children's hospital on the same campus, so highly specialised treatment is local rather than a trip to another city. But access runs through your huisarts's referral and through whether your insurer contracts that hospital — a natura policy with Erasmus MC out of network can leave you paying a share. Check the contracted-hospital list before choosing a policy, not after.

Common mistakes to avoid

  • Waiting for HR to arrange health cover that no Dutch employer provides, and blowing the four-month deadline.
  • Paying more for a basisverzekering in the belief it covers more — it is legally identical everywhere.
  • Assuming GP care being free means the tests and referrals it generates are, when those hit the €385 excess.
  • Leaving the huisarts search until you are ill, then finding every practice in your postcode is closed to new patients.
  • Choosing a natura policy without checking whether Erasmus MC and your nearest hospital are contracted.
