You need proof of citizenship or OHIP-eligible immigration status, proof of Ontario residency, and proof of identity. The accepted-documents list is specific, and being one document short is the most common reason a trip to ServiceOntario is wasted.

Apply in person at a ServiceOntario centre
Applicants over 16 generally must apply in person; this is not an online transaction. The region's centres are busiest around the January, May and September term starts, when several thousand students arrive at once — go in the quiet weeks between.

Take the coverage immediately — do not buy three months of private insurance
Eligible applicants are covered from enrolment. The three-month wait was abolished in 2020 and a great deal of published advice, and word of mouth, never caught up. The photo card arrives by post afterwards; the coverage does not wait for it.

Learn the new hospital names, because the old ones are what you will be told
Grand River Hospital and St. Mary's General merged on 1 April 2025 into the Waterloo Regional Health Network. The former Grand River K-W campus is WRHN at Midtown, St. Mary's is WRHN at Queen's Blvd, and Freeport is WRHN at Chicopee. Both Midtown and Queen's Blvd run emergency departments. Neighbours, taxi drivers and older web pages will all use the previous names for years.

Register to be matched with a family doctor on day one
Ontario runs a provincial service matching people without a family physician to one taking patients. Register when you arrive rather than when you first need care — your position depends on when you joined, and the shortage runs across the province.

Go to the Region for immunisations, vaccines and public health, not the City
Region of Waterloo Public Health handles immunisation records, school vaccination requirements, sexual health clinics, travel advice, food-premises inspection and outbreak response for all seven municipalities. It also runs the paramedic service. Kitchener's own website will not have any of it.

Arrange cover for what OHIP excludes
Prescription drugs for working-age adults, dental, vision and physiotherapy sit largely outside OHIP. Employer benefit plans normally fill these, which makes the benefits package a real part of a job offer — and in a region with this much software and insurance employment, those plans are often unusually good.

Things most newcomers don’t know

There is no three-month wait, and newcomers still pay for private cover they do not need.

The waiting period was removed from the regulations in March 2020 and eligible residents are covered from enrolment. It is one of the most persistently mis-reported facts about moving to Ontario — older guides, relocation agents and friends who arrived before 2020 all describe the old rule. Three months of unnecessary travel insurance is a several-hundred-dollar mistake made on stale advice.

The hospital merger is recent enough that half the information you find is under the old names.

The Waterloo Regional Health Network was formed on 1 April 2025 out of Grand River Hospital and St. Mary's General Hospital, and the sites were renamed rather than merely rebranded. Anything written before that — including the wayfinding in your own head after asking a neighbour — uses the old names. The practical risk is arriving at the wrong site during an emergency, so learn which of Midtown and Queen's Blvd is closer to you and what each does.

The new joint hospital is a 2034 story, not a 2026 one.

A single replacement hospital is planned for lands near the David Johnston Research and Technology Park in Waterloo, and it is not expected to be operating until around 2034. It gets talked about locally as though it were imminent, which it is not — you are dealing with the existing three sites for the whole of a normal relocation horizon. Do not weight it in a decision about where to live.

There is a physical-presence test, and a long trip home in your first six months can cost you.

You are generally expected to be in Ontario for at least 153 of your first 183 days. Newcomers who go back to settle affairs, or take an extended visit after arriving, can put eligibility in question at exactly the point they are relying on it. If a long absence is unavoidable, understand the rule before you book.

Common mistakes to avoid

Buying three months of private cover on the basis of the abolished waiting period.
Turning up at ServiceOntario without one document from each of the three required categories.
Searching for 'Grand River Hospital' or 'St. Mary's' and following directions written before April 2025.
Looking for immunisation and public health services on kitchener.ca rather than the Region's site.
Waiting until you are ill to register for a family doctor.
Assuming prescriptions, dental and vision are covered — for working-age adults they largely are not.
Travelling extensively in your first six months and breaching the physical-presence expectation.
