The SIP card, and the province where hospitals changed hands

The sequence is padrón, then social security registration, then the SIP card issued by the Generalitat Valenciana. Public care is good and free at the point of use once you are in it. What is specific to Alicante province is that several health departments ran for years under public-private concessions — Torrevieja and Dénia have since returned to direct public management, Elx–Crevillent's concession was extended — so which department your address falls into determines who runs your hospital.

The Comunitat Valenciana divides Alicante province into health departments, and this is the province where the public-private concession model was concentrated. Torrevieja's department returned to direct public management in 2021 and Dénia's followed at the end of its concession; the Elx–Crevillent department covering Vinalopó had its concession extended instead. Care remains public and free at the point of use throughout, but staffing, waiting patterns and systems differ. Ask which department an address falls into before committing to it.

The S1 form, not a private policy, is what covers UK pensioners here.

A large share of the province's older residents are UK state pensioners, who are entitled to Spanish public healthcare funded by the UK through the S1 route rather than through Spanish social security contributions. It is requested from the UK before or shortly after moving and registered with the Instituto Nacional de la Seguridad Social here. People who do not know it exists buy private cover they never needed, and people who assume it is automatic arrive at a health centre with nothing.

Residency alone does not give you healthcare — social security does.

The chain is padrón, then social security registration, then the SIP card. Newcomers assume that legal residence is sufficient and turn up at a centro de salud with a TIE expecting to be seen. Entitlement in the ordinary case is contributory, which means employment or autónomo registration is what opens the door. There are non-contributory routes, but they are narrower than people expect.

The card is Valencian, and the coast's summer surge is real.

Health is devolved, so the SIP, the system and the app are regional — move to Andalusia or Madrid and you re-register there. Locally, the resident population of the Costa Blanca is a fraction of its August one, and pressure on health centres, emergency departments and pharmacies in high season is genuine. Get repeat prescriptions and scheduled reviews done between October and May rather than in the middle of the season.
