Voluntary IESS affiliation is genuinely cheap and most foreign residents never look at it.
Anyone resident and not in an employment relationship can affiliate by declaring a contribution base and paying 17.60% of it, with the floor at the 2026 basic salary of USD 482 — USD 84.83 a month. That buys access to the IESS health network, and a spouse can be added for 3.41% of the base. Retirees and remote workers who default straight to expensive international insurance frequently do not realise a public option is open to them at that price.

Altitude is a two-week problem and a permanent one at the same time.
The acute part — headaches, breathlessness, insomnia, sharply reduced alcohol tolerance — resolves within a week or two as your body adapts. The permanent part is ultraviolet exposure: at the equator and at 2,850 metres, UV levels are among the highest recorded in any city, and they are high on cool, cloudy days when nothing feels dangerous. Skin damage here accumulates far faster than at sea level and daily sunscreen is a local habit for good reason.

Some smaller employers treat it as optional or delay it to the end of a probation period.
