Mexican employers are legally required to register new hires with IMSS within days, whatever private cover you already hold. Ask HR to confirm your NSS and which Unidad de Medicina Familiar you have been assigned to, then register on IMSS Digital so you can book appointments and pull your own records.

Buy voluntary IMSS if nobody employs you here
Seguro de Salud para la Familia is the self-purchased public option. Apply with CURP, resident card and proof of address, pay the annual premium at a bank — it is annual and in advance, with no monthly option — and cover begins on the first of the following month. It is cheap and genuinely useful as a floor, but read the exclusions before relying on it.

Add a private policy and choose the hospital tier deliberately
GNP, AXA, MetLife and Mapfre write domestic policies; the premium tier decides which hospitals you can use. In Guadalajara that means choosing between the mid-tier network and the top hospitals — Puerta de Hierro, Country 2000, San Javier. Expect medical underwriting and permanent exclusion of pre-existing conditions.

Know that Guadalajara's public teaching hospitals are genuinely good
The Hospital Civil de Guadalajara — Fray Antonio Alcalde and Juan I. Menchaca — is one of Mexico's oldest and largest teaching hospital systems, tied to the Universidad de Guadalajara. For complex specialist care it is a real option rather than a last resort, which is not true of the public system in every Mexican city.

Save the right emergency numbers before you need them
911 is the national emergency line. In Guadalajara it dispatches Cruz Verde, the municipal paramedic and emergency clinic network run by the city councils, alongside Cruz Roja. Cruz Verde will take you to a public facility — if you want a specific private hospital, you need that hospital's own ambulance number saved separately.

Use pharmacy consultorios for anything minor
Farmacias Similares and Farmacias del Ahorro run walk-in doctors' offices attached to the shop for roughly MXN 50–80, no appointment and no insurance. For a chest infection, a UTI or a prescription refill this is what locals do, and it works from your first day in the country.

Things most newcomers don’t know
Cruz Verde is a Guadalajara institution that does not exist in most of Mexico.
The metro municipalities run their own network of emergency clinics and ambulances, staffed by municipal paramedics and free at the point of use. It means the response to a street accident here is generally faster than in comparable Mexican cities. It also means the default destination is a public facility, so if you hold private cover you should save your hospital's own ambulance line rather than assuming 911 will deliver you there.

Voluntary IMSS permanently excludes major pre-existing conditions, not merely defers them.
Seguro de Salud para la Familia carries waiting periods for many conditions and outright, permanent exclusions for others — malignant tumours, congenital conditions, chronic degenerative disease, HIV, psychiatric illness and addiction among them. IMSS also declines applicants who already have one of those conditions, and will refuse treatment later if you did not declare it. People buy it as a safety net and discover the net has a hole exactly where their diagnosis sits. If you have a serious condition, it is not a substitute for insurance.

Private premiums are cheap by foreign standards but underwriting is strict, and it gets stricter with age.
A domestic Mexican policy costs a fraction of a US plan, which is why nearly every relocating professional buys one. What newcomers underestimate is that insurers medically underwrite at entry and permanently exclude anything you already have — and that new policies become harder and dearer to obtain in your sixties. Buying while healthy, and ideally before you arrive, is worth far more than shopping on price later.

The Hospital Civil changes the calculus for complex care in Jalisco.
Guadalajara has one of Mexico's largest public teaching hospital systems, attached to a major medical school, with specialist units that genuinely lead nationally in some fields. Elsewhere in Mexico the private-versus-public decision is easy; here, for a complicated diagnosis, the public teaching hospital may hold the deepest expertise in the state. Knowing that exists is worth more than another comparison of private premiums.
