EPS, prepagada, and what 2,640 m does to your first fortnight

Colombia's contributory health system runs through EPS insurers. Employees contribute 4% of salary with the employer adding 8.5%; the self-employed pay the full 12.5% of a declared income base that cannot fall below one minimum wage, which for 2026 means a floor of roughly COP 219,000 a month. Affiliation requires a cédula, and some visa classes — notably M-Pensionado and several V categories — cannot affiliate at all and rely on private cover. Most foreigners who can, hold EPS plus a medicina prepagada plan on top for fast specialist access. The sector is in the middle of a contested reform with several large EPS under Superintendencia intervention, so check that the insurer you pick is actually operating normally in Bogotá before you sign.     EPS affiliation in the contributory regime requires a cédula and an income base. Formal employees are enrolled by their employer. The self-employed enrol directly and pay 12.5% of declared income.     Choose an EPS that is actually functioning in Bogotá

    This is the step that needs current information rather than a list from a blog. Several of the largest EPS have been placed under Superintendencia Nacional de Salud intervention during the sector's ongoing reform, and 2026 rules also restrict an EPS from operating in a department where it holds too small a share of affiliates. Before enrolling, confirm with the insurer and with the Supersalud that it is operating normally in Bogotá and that the clinics you want are in its network.

    Add medicina prepagada or a complementary plan on top

    Prepagada is the upper private tier and is sold as a supplement — you must keep an active EPS underneath it. It buys direct specialist access without a GP referral, short waits, private rooms and the best hospitals. Sura, Colsanitas and Coomeva are the main providers, priced by age from roughly COP 200,000 a month in your twenties to COP 700,000 in your fifties.

    Know which hospitals you would actually want

    Bogotá has Colombia's deepest concentration of hospital medicine. The Fundación Santa Fe de Bogotá, Fundación Cardioinfantil, Hospital Universitario San Ignacio, Clínica del Country and Clínica Marly are the private names foreigners use, with international patient offices at several. Public emergency care runs through four Subredes Integradas de Servicios de Salud. Emergencies are 123 nationwide.

    Get the yellow fever vaccine before you travel down from the plateau

    Bogotá itself is above the altitude at which yellow fever, dengue and malaria transmit. But Colombia has been under a yellow fever health emergency declared in April 2025, vaccination at least ten days before entry is required for national parks, and the vaccine is being requested at land transport terminals for travel to risk areas. If you plan weekends in the Llanos, Tolima, the coffee region or the coast, get vaccinated in Bogotá first and check the current MinSalud position.

Things most newcomers don’t know

Bogotá's altitude puts it outside the dengue, yellow fever and malaria transmission zone that covers most of Colombia — but a weekend in Villavicencio or on the coast puts you right back inside it.

These diseases need Aedes and Anopheles mosquitoes that do not establish at 2,640 m, which is why Bogotá residents can skip repellent entirely at home. The trap is the four-hour drive down to the Llanos: Colombia's yellow fever emergency, declared in April 2025, has produced cases specifically in unvaccinated travellers from Bogotá and Cundinamarca who went to risk areas.

Your visa's insurance requirement and EPS are different things, and EPS usually does not satisfy the visa.

Digital-nomad and visitor visas require all-risk private insurance valid in Colombia for the full visa period and expressly covering medical repatriation. Ordinary travel insurance is rejected and EPS is not what the Cancillería is asking for. People arrive proud of having affiliated to a cheap EPS and then fail the renewal condition they never read.

Medicina prepagada is a top-up, not an alternative: you cannot hold it without an active EPS underneath.

The private tier is legally structured as a supplement to the mandatory system, so the insurer will ask which EPS you are affiliated to before selling you a plan. Foreigners who plan to skip the public system entirely and buy 'good private cover' discover the two are coupled, and the combined bill is still modest by international standards.

  • Assuming EPS satisfies your visa's health-insurance condition — the digital-nomad and visitor visas require all-risk private cover including repatriation.
  • Trying to buy medicina prepagada without an active EPS underneath it, which insurers will not do.
  • Choosing an EPS from an out-of-date list without checking its current status with the Superintendencia Nacional de Salud.
  • Driving down to the Llanos or the coast for a weekend without a yellow fever vaccination given at least ten days beforehand.
